When referring a patient to our imaging center in Cape Girardeau please submit the following items:
- A written or electronic request for the exam being ordered.
- Any prior exam information that relates to the exam being ordered. (i.e. If the patient is being referred for a mammogram and had prior mammograms at other facilities these images will be needed for comparison.)
- All patient insurance information.
Please note: Subject to insurance providers, your assistance may be needed in the precertification process as most insurers now require pre-authorization for advanced medical imaging such as MRI or CT.
Due to contract requirements with the insurance companies, pre-authorization is required to be obtained by the referring physician office.
