If you have been referred to our Cape Radiology Group imaging location, completing the following forms and bringing them with you the day of your appointment will expedite the registration process.
Assignment of Benefits (New and existing patients)
Patient Registration Form (Existing patients)
MRI Patient Questionnaire (Complete if scheduled for a MRI)
Release for Prior Mammogram Exams (Complete if prior mammogram performed elsewhere)
You will need the Adobe Reader to view and print these documents. 
Nursing home patients scheduled for an MRI: if the legal guardian is unavailable or not present for the exam, completing the MRI Patient Questionnaire in advance will lessen the chance of exam cancellation.
