If you have been referred to our Cape Radiology Group imaging location, completing the following forms and bringing them with you the day of your appointment will expedite the registration process.
Assignment of Benefits (New and existing patients)
Patient Registration Form (Existing patients)
MRI Patient Questionnaire (Complete if scheduled for a MRI)
You will need the Adobe Reader to view and print these documents. 
Pediatric patients scheduled for an MRI: if the legal guardian is unavailable or not present for the exam, completing the MRI Patient Questionnaire in advance will lessen the chance of exam cancellation.
Any patient under the age of eighteen must have the person financially responsible for them sign the Assignment of Benefits form.
