How To Refer A Patient

Referring a patient to Cape Radiology Group is simple. To help us schedule your patient efficiently and provide the highest-quality care, please include the following information with each referral.

Step 1 Submit an Imaging Order

Provide a written or electronic order that includes:

  • Requested imaging exam or procedure
  • Clinical indication or diagnosis
  • Relevant symptoms or history
  • Referring provider information
  • Preferred location if applicable

Step 2 Send Relevant Prior Imaging

Prior imaging studies and reports are important for comparison and diagnostic accuracy whenever available.

Examples may include:

  • Previous mammograms
  • Prior CT or MRI studies
  • Ultrasound reports
  • Outside imaging records


Providing prior studies can help reduce unnecessary repeat imaging and improve interpretation accuracy.

Step 3 Include Insurance Information

Please include all current patient insurance information at the time of referral, including:

  • Insurance carrier
  • Policy and group numbers
  • Authorization details when applicable

Step 4 Pre-Authorization Requirements

Many insurance providers require pre-authorization for advanced imaging studies, such as:

Due to insurance contract requirements, pre-authorization is generally required from the referring physician’s office before scheduling.
