Imaging Services For Nursing Home Patients
Coordinated, Compassionate Imaging Care For Long-Term Care Residents
Cape Radiology Group proudly provides imaging services to nursing home and long-term care facility patients, supporting caregivers and healthcare facilities throughout Southeast Missouri, Kentucky and Southern Illinois with safe, efficient, and compassionate imaging for all our patients.
We understand that coordinating medical imaging for long-term care patients often requires additional planning, communication, and support. Our team is committed to helping make the process as smooth and stress-free as possible for patients, caregivers, legal guardians, and facility staff.
Preparing For Your Imaging Appointment
To help streamline the registration and imaging process, we recommend completing all applicable patient forms before the appointment whenever possible.
The MRI Patient Questionnaire helps our team review important medical history and safety information prior to your exam. This information is essential for determining MRI compatibility and ensuring a safe imaging experience.
Patients scheduled for exams other than MRI typically do not need to complete forms in advance unless otherwise instructed by our team.
MRI Patients & Legal Guardian Requirements
For nursing home patients scheduled for an MRI, it is especially important to complete the MRI Patient Questionnaire before the appointment, whenever possible.
If the patient has a legal guardian who will not be present during the exam, completing the MRI questionnaire in advance may help prevent delays or cancellation of the procedure.
Because MRI exams involve strong magnetic fields, our team must review important medical and safety information prior to imaging.
What Nursing Home Staff & Caregivers Should Bring
To help ensure a smooth appointment experience, please bring:
- Completed MRI Patient Questionnaire (for patients scheduled for an MRI exam)
- Driver’s license or valid photo ID if available
- Insurance card(s) or insurance information
- Physician referral or imaging order
- Current medication list, if available
- Relevant medical history or care instructions
- Contact information for legal guardian or responsible party, if applicable
Prior Imaging Records
If imaging studies were previously performed at another facility, please notify our team in advance so prior records can be requested if needed.
