Patient Registration & New Patient Forms
Prepare For Your Visit To Cape Radiology Group
At Cape Radiology Group, we want your imaging appointment to be as smooth, efficient, and stress-free as possible. For patients scheduled for an MRI exam, completing the MRI Patient Questionnaire before your appointment can help streamline the check-in process and ensure important safety information is reviewed prior to your visit.
Whether you’re visiting for diagnostic imaging, breast imaging, or an interventional radiology procedure, our team is here to guide you every step of the way.
MRI Patient Questionnaire
Required for patients scheduled for an MRI exam to ensure MRI safety and compatibility. Patients scheduled for imaging exams other than MRI typically do not need to complete forms in advance unless otherwise instructed by our team.
When Should Forms Be Completed?
To streamline your appointment experience, we recommend completing all required forms before your visit.
Before Your Appointment
Please complete and print forms:
- Before your appointment, whenever possible
- Before arriving at the imaging center
- For every patient visit requiring updated information
Arrival Time
Patients should arrive approximately 30 minutes before their scheduled appointment time to allow for registration and check-in.

To help ensure a smooth registration process, please bring the following items with you on the day of your appointment:
- Driver’s license or valid photo ID
- Insurance card(s) or insurance information
- Completed registration forms

Insurance & Prior Authorization Information
